PACKER CENTENARIAN QUESTIONNAIRE

Your alumni association would like to honor our alumni who are 100 years or older. If you went to South High between 1928 and 1940, please complete and submit the questionnaire below. If you know someone who fits this description, please tell them about this project.

Submission of the questionnaire implies permission for us to use the content submitted in both printed and social media communications. The amount of information published in the Alumni Tooter will be based on available space.
